Do cooling pillows actually work?
Some of them, by one mechanism. The others work for about as long as it takes you to get comfortable.

Cooling pillows work when the fill is open — shredded foam, latex or buckwheat — because air can move through and be exchanged as you shift. They do not work when the claim is a gel layer or a cold-touch cover on a solid block: those feel cold for a few minutes and then match your skin temperature.
The pillow category has more marketing per dollar than anything else in bedding, largely because a pillow is a cheap enough impulse buy that nobody investigates. Here is what each claim is doing physically.
Claims that work
Open or loose fill
Shredded foam, latex and buckwheat all leave interconnected air spaces. Because you move during the night, those spaces get pumped and the air in them exchanges with room air. This is convection and it does not saturate, wear off or wash out. It is the only mechanism in a pillow that works all night, every night.
Absorbent cover fabrics
Viscose, lyocell or cotton pull sweat off your skin into the fiber. Your head is where sweat concentrates most, so this is genuinely useful, and it keeps working as long as the fabric is clean and dry — which is an argument for owning several cases and changing them mid-week.
Spacer-knit (3D or honeycomb) covers
A fabric woven with two faces joined by vertical yarns, holding an open gap. It is a structural airflow layer rather than a surface treatment, so unlike a coating it does not stop working after five minutes or after twenty washes.
Claims that do not survive the night
Gel layers and gel infusion
A gel layer raises the pillow's heat capacity, meaning it takes marginally more energy to warm up, and it conducts heat away from your skin quickly at first. Both effects are real and both are bounded: once the gel reaches your skin temperature, no further heat flows into it, and there is nothing behind it to carry that heat away. Minutes, not hours.
Cold-touch (Q-Max, “ice silk”, “arc-chill”) covers
Same physics, applied to the surface. Q-Max is a legitimate measurement of instantaneous contact coolness — it is just a measurement of the first seconds, which is precisely what the marketing does not say. Genuinely helpful for falling asleep. Not a night-long cooling system.
Copper, charcoal, bamboo charcoal and graphite
Copper and graphite raise the thermal conductivity of foam slightly, so heat spreads through the slab a little faster — which still does not give it anywhere to go. Charcoal is included for odor control, which is a hygiene benefit and not a thermal one. None of these changes the fundamental behavior of a solid block.
“Ventilated” on a solid foam pillow
Real, and modest. Drilled channels restore a convection path an otherwise sealed block does not have, so a ventilated solid pillow beats an unventilated one. It does not beat shredded fill, which is all channel.

The honest verdict
A $20 shredded-fill pillow with a $15 viscose case will outperform a $65 gel memory foam pillow for a hot sleeper, essentially every time, and it is adjustable as well. That is a strange sentence for a page that earns a commission either way, and it is what the mechanisms say.
If you have done the pillow and the case and your head is still hot, the problem is probably not the pillow — check the room and the top layer via what to fix first.
